  5. Caste and income certificate RD numbers in the name of the student
  6. UDID identification number if the student is disabled
  7. District, taluk, assembly constituency, and student residence address
  8. Student or university registration number
  9. E-attestation number of related documents if applicable
  10. Hostel details if applicable
